New Product submission
When you have a new product, we need the following filled out PRIOR to listing your new product on ANY sites (ecommerce, Amazon, wholesale, etc). We must have this complete before we can ship any product from our facility or receive new product. If you have any questions about this form, please email or call the office for help.
